Other Insurance companies: I will be happy to provide you with a claim form (CMS 1500) for services you have paid for, but I am not able to assist you in submitting claims to other insurance companies.
I am happy to take your payments by cash, check, and PayPal. I require payment at the beginning of each session (except for portions covered by insurance), or the session will not occur. This is to reduce the time I have to spend managing each clients billing. With digital payments, it is easy to wait until 5 minutes before the session to make your payment.
You should come ready to describe what goals you would love to accomplish with therapy. This usually means explaining your primary reason for seeking therapy (the problem), how your life is negatively affected by it, and finally, how solving these issues would make your life different (your desired goal).
The first session creates an opportunity for you to get to know your counselor. A good match is essential. You will be sharing personal and intimate details about your struggles and challenges, and it is important to be comfortable with, and in time learn to trust, your counselor. I do offer a free 15-minute consultation to help us both get an idea of what it will be like to work together. This is available in person or over the phone.
In order to take full advantage of our first full session, I strongly urge you to download the intake form and complete it prior to our first appointment. If you chose not to complete the form before your visit, we will have to complete it during our first session together.
At the end of our first session, we will together make the determination of whether or not to continue with therapy together.
Each individual will have different therapeutic needs, however, in general CBT for anxiety and/or depression issues will last 3 to 6 months, assuming sessions are every week AND assuming the client completes homework outside of our sessions together. CBT homework is normally not intellectual work but can be emotionally taxing. Not completing this homework will slow your progress.
Specific phobias may be resolved in as little as 2 months depending on the severity.
Therapy often involves sharing sensitive personal information. I will keep confidential what you share as part of our counseling relationship including your records, progress notes, and even appointment scheduling. Additionally, I am legally and ethically bound to obtain a signed authorization from you to release any information. This is required before I communicate with others such as medical or mental health professionals, family members, or anyone else.
However, there are legally defined exceptions to confidentiality. These exceptions include:
(a) You direct me in writing to disclose information to someone else, or
(b) It is determined you are in imminent danger of harming yourself or others, or you tell me of specific instances of harm to others (including abuse, neglect, or exploitation (actual or suspected) of a child, a person with disabilities, or an elder). Harm to others often includes physical threats of violence, abuse, or through knowingly spreading life-threatening communicable diseases, or
(c) I am ordered by a court to disclose information.
It is also important for you to know that if I see you in public, I will protect your confidentiality by greeting you only if you greet me first.
If at any time you have any questions regarding confidentiality, you should bring them to my attention.
I offer virtual and in person sessions. My virtual sessions are Tuesdays and Wednesdays. I now offer in person sessions 2 days per week on Thursdays and Fridays. For in-person sessions, I have these two office locations.
On Thursdays I am at 1405 Hillsborough St, Raleigh, NC 27605, near NC State campus. There is a parking lot as well as on street parking.
On Fridays I do an office rental located above Perry's Steakhouse at 4509 Creedmoor Rd., Raleigh, 27612. This is at the corner of Glenwood Avenue and Creedmoor Road next to Crabtree Valley Mall, and across the street from Macy’s Department Store. There is a free parking deck/garage at this location.
